Preparing for your office visit:

• To schedule a TELEMEDICINE visit contact office, email request via patient portal, send message on adjacent link or schedule online on homepage
• To schedule your in office appointment contact office, email request via patient portal or send message on adjacent link or schedule online on homepage
• Avoid lotions, oils and perfumes/fragrances/sprays on the day of your visit per health guidelines 
• Respond to your Email invitation to join our patient portal and complete your patient forms prior to your visit
• If you are unable to access the patient portal, then arrive 30 minutes prior to your appointment
• Bring your Insurance card, Photo ID or Drivers License, current medications and any records
  
• Manage payment via Patient Portal
