Preparing for Your Office Visit

  • Schedule OFFICE or TELEMEDICINE visits: call our office at 248-476-7650, email a request via the Patient Portal, send a message on the adjacent link or use the BOOK ONLINE NOW button on the home page
  • Avoid lotions, oils, and perfumes/fragrances/sprays on the day of your visit per health guidelines
  • Respond to your email invitation to join our Patient Portal and complete your patient forms via the Patient Portal or use the Patient Intake button prior to your visit on the home page
  • If you are unable to access the Patient Portal, then arrive 15 minutes prior to your appointment
  • Bring your Insurance card, Photo ID or Driver's license, current medication list and any pertinent medical records to your visit
  • Platform patients may scan their Insurance card and Drivers License directly from the platform site
  • Manage payment via your Patient Portal or call our office
